U.S. Embassy offers information sessions on Social Security in Quito and Cuenca

U.S. Social Security information sesions: Federal Benefits Services Visit to Ecuador, March 10 to 13, 2025
Location: Quito and Cuenca
The U.S. Mission in Ecuador is pleased to announce that representatives from the Regional Federal Benefits Unit will visit Quito and Cuenca to offer services for U.S. Social Security and other federal benefits.
Quito – U.S. Embassy

- Monday March 10, 2025, from 8:00 AM to 4:00 PM
- Tuesday March 11, 2025, from 8:00 AM to 3:00 PM

If you only wish to submit your Foreign Enforcement Questionnaire SSA 7162/7161 form for 2024, NO appointment is required. You may drop the form during service hours at the above locations.

You must bring original documents as well as a photocopy of each document required.
How to prepare for your appointment:
Please keep in mind that you will not be granted entry to the Embassy with purses, bags, backpacks or electronic devices. For more information regarding entry requirements, please review the security guidelines on our website.
- Application for Social Security Benefits
For each applicant, bring an original and a copyof the birth certificate and valid U.S. passport. If applying for auxiliary benefits or survivor’s benefits as the spouse of a deceased beneficiary, bring the original and copy of the marriage and death certificate.
| - Application for Social Security Number Card
For each applicant, bring an original and a copyof the birth certificate, valid U.S. passport, and completed Application for a Social Security form SS-5FS (PDF 125KB). We recommend using a U.S. address to receive the card, due to the significant delays with the Ecuadorian mail system.
If you are applying for a minor under 12 years old, also bring original and a copy of the parent’s valid ID.
If the applicant is 12 years old and older, also bring the additional documentation.
| - Foreign Enforcement Questionnaire (FEQ)
Please bring the corresponding FEQ form correctly filled out and a copy of your valid ID.
FBU will accept the “proof of life” forms. These were mailed this year to individuals with Social Security Numbers ending in 00 through 49 and who have a mailing address registered in Ecuador. If you didn’t return this form by January 2025, your benefits have been or will be suspended in February 2025.
